Patient Smile Rewards
Creating a beautiful smile requires both a commitment from us and cooperation from you, our patient. Our Rewards Program was created as a way to recognize all of your hard work by awarding you fantastic prizes as you progress through your orthodontic journey. During each appointment, points will be awarded for outstanding cooperation.
How do I get a Smile Rewards Card?
Cards are issued for all new patients at the start of treatment. Existing patients can transfer to the Smile Rewards program by talking with any of our team members.
How do I earn points?

How do I earn Smile Rewards for visiting my regular Dentist?
Download and print the form and ask your Dentist/Hygienist to complete and sign it. Bring it with you to your next appointment at Bunker Family Orthodontics, and the points will be added to your card. Download the certificate here.
How do I see how many points I have?
You can view your points at any time and redeem them on our rewards hub. Enter your 16-digit rewards number and 4-digit password to check your current points.
How do I redeem my Smile Rewards for a prize?
Trade in your Smile Reward points for great prizes either in person at your next visit or online on our rewards hub.
What rewards can I get with my points?

What happens if I lose my Smile Rewards card?
Don’t worry, your points are safe! Let a team member know you’ve lost your card, and they will get you a new one!
